Clinical nursing care

At Guardian Angels at Home, we understand that as we get older, we have more health conditions that require chronic monitoring and management. With a Registered Nurse on hand and working alongside your GP, this service is provided with knowledge and understanding. This service includes the management of chronic diseases such as diabetes, arthritis, asthma, dementia, osteoporosis, strokes, coronary heart disease, chronic obstructive pulmonary disease, chronic kidney disease, and others, working in cooperation with your doctor. As well as disease management, the service assists with catheter and stoma care, education, wound care management, medication management, organisation of medical and allied health referrals, and other services.

Case management

If you have or are looking to apply for a home care package or NDIS plan, we can manage these packages for you. These include providing the required ongoing assessments to ensure that the most appropriate and available services are provided to suit your needs, liaise and organise with service providers, medical and allied health professionals, organise services, personal care and appointments on your behalf, financial support and management of the care package, and ensure that you are kept up to date with the progress or changes to your care package.

Wound care management

Our experienced Registered Nurse, with your doctor, develop a wound care management plan that is specific to your wound care needs. These wounds may include leg and foot ulcers, pressure injuries, post-surgical wounds, and acute wounds such as cuts, skin tears or abrasions. Our goal is to ensure that your care needs are right for you, provided in an adequate time frame, so that you can stay at home and enjoy a good quality of life.

Medication management

We understand that it is important that your health is a priority and that includes ensuring that medication is taken as prescribed. Our staff are available to provide support, organise webster packs and assistance with monitoring your medication, empower you to maintain your own independence with your cares, and provide peace of mind for everyday living.

General observations monitoring

It is important to ensure that we monitor our health, especially as we get older. Our care staff are trained to provide adequate knowledge and skilful care when providing general observations to our clients. These observations include monitoring blood pressure, heart rate, temperature, blood sugar levels, oxygen saturation levels, height, and weight, in accordance with any doctor recommendation and requirements, and client’s personal needs.

Personal care assistance

We pride ourselves in providing respectful, dignifying and quality personal care services, and to encourage independence when attending to everyday personal activities. Our care team provides support and assistance with personal care activities such as showering, dressing, grooming, personal hygiene, and toileting and incontinence care.

Shopping assistance

Sometimes having to do grocery shopping can turn into a chore. With our team, we can take you to the supermarket or shopping centre, assist with pushing the shopping trolley or carrying the shopping basket so that you are able to peruse at your own pace. We can support you with finding all the items on your shopping list, reach the high up items, pack, and unpack the shopping, and assist with online shopping if that is your preference.

Meal preparation

Eating healthy and nutritious meals are important to maintain a healthy lifestyle. Our staff can be available to support and assist with the preparation of meals with you in your home, or if required, we can prepare the meals for you. This can include meals to be prepared in advanced for the week as well.

Domestic assistance

A helping hand around the home can help make your life easier and worry free. Our staff can assist with provided domestic services so that your home is looking nice, these can include general house cleaning, light cleaning, changing of bedsheets, laundry, and other day to day tasks.

Pet care and minding

We understand that our four-legged friends are as much a part of the family as the people. So, we offer support and assist with caring for your furry friends, whether it is walking them, feeding them, watching them if you are away or in hospital, organising vet or grooming services. Some pet services may require further safety assessments.

Mental wellbeing and emotional support

We understand that getting older can become lonely. Providing one-on-one support to clients while attending cares or even just to stop in for a chat and a cuppa, our staff are on hand to ensure that your mental and emotional wellbeing are supported and monitored. As well as providing mental and emotional support, Guardian Angels at Home focus on providing cognitive programs and activities for dementia prevention which include Brain training and Brain wise.

Social support

Attending social activities and groups is a great way to make new friends, maintain old friendships, have fun, and enjoy new experiences. At Guardian Angels at Home, we understand the importance of feeling connected and valued, and being a part of the community. From one-on-one board games to social group activities with likeminded people, we are happy and motivated to work with you to find people and hobbies and interests that suit your personality and passions.

Respite

We understand that it can be stressful and tiring caring for someone fulltime and that sometimes we need a break or a day to ourselves. We also understand that things can happen such as a trip to hospital and we worry that our spouse is unable to care for themselves properly. Our trained staff are available to provide respite services for those occasions for you or your spouse/carer when they are needing a break due to carer’s stress or visiting family/friends or needing to be admitted in hospital. This may include day respite or overnight respite.

Transport services

We understand that travel expenses can be expensive and difficult as we get older, but we also understand that providing transport helps to maintain independence with our everyday activities and living life your way. Our staff can make sure that you are able to get to your medical appointments, social activities with family and friends, a trip to the shops, or a trip to the library.

Safety and equipment management

Safety is one of our utmost priorities and we ensure that every client is well equipped to safely maintain their independence in the home. This may include organizing equipment hire such as mobility and motorized aids, personal alarms or call assist alarms, shower chairs, security safety devices, and more. Ongoing assessments ensures that all necessary safety equipment is obtained and appropriately managed and maintained.

Maintenance management

As you get older looking after you home becomes harder and harder, cleaning those pesky gutters and mowing the lawn especially.  Let Guardian Angels at Home organize home maintenance and repairs for you.
